When you start a T20 batch it's a good idea to dedicate one post where you can have an index of the entire 20 trades.
Below I have given a sample/template of how to record your trades.
The First post should contain your Trading Plan Copy and a bit of introduction about you. Record each trade in a separate post/entry.
Template for T20 Journal Entry
You can save a link to this page so you can copy. Or copy this on to a Notepad (windows) and save it onto your desktop. You can then later copy and paste when you create a new post for recording trades and fill it in. Don't forget to attach at least one screenshot and/or video.

5. Questions Before Trade is Taken:
a) How do I feel before setting the order?
[explain in one or two sentences how you feel - refer to your emotions) [This question is about your emotional response]
b) What is my intuition about this signal or setup?
[e.g. What is your intuition about this trade? Do you think its a good setup? Do you think it will be a winner? How do you feel internally? Is the technical analysis correct? Is the market really in a good position to go in this direction?
c) Is this technically The Setup 1?
[This question is entirely focused on the technical analysis side. Is this really a perfectly good setup 1? Is M1 creating the proper setup 1 pattern? Is the M5 Chart (or the 5XHTF) creating the correct corresponding pattern? Gator is open and there is a 4-5 candle pullback? etc]
6. Question After The Trade has been closed:
a) How Do I feel after closing the trade (or end of the session for Day Traders)?
b) Have I done well and why?
c) Have done badly and Why?
d) What Could I have done differently?
7. DS For this Trade:

Example of a Completed Journal Entry:
1. Trade Number: [Enter Trade Number e.g. B1-T1 (Batch 1 Trade 2) ]
2. Date/Time: [Enter Date of the Trade]
3. Symbol/TF: [Enter Pair Symbol e.g. EUR/USD H1 ]
4. Signal Type / Direction : [Enter the type of signal and direction e.g. Setup 1 BUY]
5. Comments and Feelings at the Time of Taking The Trade: e.g. I saw the signal at the London Open. There is no news at this moment. I feel that the euro has been weak this week. All euro pairs are moving. I'm feeling a bit nervous as it's my first trade but I am here to train for discipline. So I will try to be as objective as possible.
6.Post Trader Questions:
a) I was feeling a bit nervous during the trade, there were some levels to the left that were bothering me.
b) I held on to the trade convinced that I have followed by trade plan and that ATM is working. I have taken the correct setup and so I will let the ATM manage the trade. I will not intervene so that I can grab a nice lean DS 1
c) Slight mistake at the time of entry went in a bit late but I'm aware of it and will be careful next time.
d) When it was correct time to exit, I hesitated a bit and got a slightly worse exit but that still does not break the rule of my trade so it's ok. I will try to exit near the levels next time.

End of Batch Review
Recording your trades will make you a better trader. But not without end of Batch Review Process. You will experience a great sense of achievement at the end of each batch. The 20th trade gives me great pleasure. Then comes a VERY IMPORTANT STEP.
The Batch Review:
REVIEW each Batch and record your feelings, observations and lessons you may have learnt during this batch. This step must not be ignored. This is the fruit of your labour. This is what makes you a better trader.
Use the Following Template at the end of T20 Batch 1
1. Total DS for this Batch:
2. Feelings and comments about the entire batch.
3. What have I learned about my performance?
4. What will I do differently in the Next Batch?
5. How did the market perform during this batch? Was the market more or less volatile than usual? Was some major economic events happening in the world e.g. US Elections, Brexit type of events etc? Was the market choppy?
You may add your own questions to this list.
